upgrade.report_dict()

A report dataclass as plain data for the op log (without its plan).

upgrade.report_dict(report)

Every apply_* – in the core, in tether.upgrade, in tether.experimental.registry – records its report this way, so the helper lives with the log rather than as a private name in repo.py.
